糖尿病
糖尿病は腎臓に大きな影響を与える病気です。
血糖値が高い状態が長く続くと、尿アルブミンが増え、徐々に腎機能が低下することがあります。
そのため当院では、単に血糖値を下げるだけではなく、
血糖・血圧・脂質・体重・腎機能をまとめて考えること
を大切にしています。

高血圧
高血圧と腎臓病は密接に関係しています。
血圧が高い状態が長く続けば腎臓に負担がかかり、腎機能が低下することがあります。
一方で、腎臓病が原因で血圧が高くなることもあります。
「薬を飲んでいるのに血圧が下がらない」
「薬が何種類も増えてきた」
「若い頃から血圧が高い」
という方は、腎臓内科で一度原因を確認することをおすすめします。
必要に応じて、二次性高血圧についても検討します。
脂質異常症
LDLコレステロールや中性脂肪が高い状態が続くと、動脈硬化のリスクが高くなります。
腎臓も細かな血管が集まってできている臓器です。
そのため、腎臓を守るためにも血圧だけでなく、脂質を含めた全身の血管リスクを管理することが重要です。

慢性腎臓病(CKD)
慢性腎臓病(CKD)は、腎臓の機能が低下したり、尿たんぱくなどの腎障害が続いたりする状態です。
CKDは、初期にはほとんど症状がありません。
しかし、進行すると腎機能が徐々に低下し、将来的に透析などの腎代替療法が必要になることがあります。
またCKDは、心筋梗塞や脳卒中などの心血管疾患とも深く関係しています。
だからこそ、早期に見つけて、できるだけ進行を抑えることが重要です。
糸球体腎炎
腎臓の中にある「糸球体」という血液をろ過する場所に炎症が起こる病気です。
IgA腎症をはじめ、さまざまな種類があります。
尿たんぱくや血尿をきっかけに見つかることも少なくありません。
「尿たんぱくと血尿が一緒に出ている」
「何度検査しても尿潜血が陽性になる」
という方は、一度腎臓内科で詳しく調べることをおすすめします。
ネフローゼ症候群
大量の尿たんぱくが出ることで、血液中のアルブミンが低下し、強いむくみなどを起こす病気です。
原因によって治療方法が異なるため、専門的な評価が必要です。
多発性嚢胞腎
腎臓に多数の嚢胞ができ、徐々に腎機能が低下する遺伝性の腎臓病です。
高血圧や血尿などを伴うこともあります。
ご家族に多発性嚢胞腎の方がいる場合や、画像検査で腎嚢胞を多数指摘された場合には、一度ご相談ください。

At Nagoya Progress Nephrology & Dialysis Marunouchi Station Clinic, we provide medical care by board-certified nephrologists and specialists in kidney disease.
When people hear “kidney disease,” they may imagine that a nephrology clinic is a place only for people whose kidney function has already significantly deteriorated.
But I do not believe that is the case.
We would like people to consult a nephrologist not only after they have already been diagnosed with kidney disease, but also at an earlier stage.
For example:
People who have been told that something was abnormal during a health checkup.
People with high blood pressure, high blood glucose levels, or high cholesterol.
People who have been found to have protein in their urine or blood in their urine.
People who are currently receiving treatment at another hospital or clinic but would like to consult a kidney specialist.
We encourage you to consult us at any of these stages.
Kidney disease often causes few or no symptoms in its early stages.
That is why it is important to evaluate kidney health not only after symptoms appear, but also when you simply feel that “something might not be quite right.”
When Should You See a Nephrologist?
If you have been told that something is abnormal during a health checkup or medical examination

Have you ever thought, “It’s only a slight abnormality, so I’ll leave it for now”?
A single abnormal test result does not necessarily mean that you have kidney disease.
However, repeated abnormalities may indicate an underlying kidney disorder.

Kidney disease often develops without obvious symptoms, but the following symptoms can sometimes be signs of kidney problems:

Of course, having one of these symptoms does not necessarily mean that you have kidney disease.
That is precisely why it is important to undergo an appropriate evaluation and find out what is causing the symptoms.
Major Conditions Treated in Our Nephrology Clinic

Diabetes
Diabetes can have a significant impact on kidney health.
When blood glucose levels remain elevated for a long period of time, urinary albumin may increase and kidney function may gradually decline.
For this reason, our approach is not simply to lower blood glucose levels.
We place importance on considering blood glucose, blood pressure, lipids, body weight, and kidney function together.
If you are being treated for diabetes and are wondering,
“Are my kidneys okay?”
we encourage you to consult us.
Hypertension
Hypertension and kidney disease are closely related.
Persistently high blood pressure can place a burden on the kidneys and eventually lead to declining kidney function.
Conversely, kidney disease itself can cause high blood pressure.

The number of antihypertensive medications you take has continued to increase

You have had high blood pressure since a relatively young age
we recommend consulting a nephrologist to investigate the possible underlying causes.
When appropriate, we also evaluate patients for secondary hypertension.
Dyslipidemia
Persistently high LDL cholesterol or triglyceride levels increase the risk of atherosclerosis.
The kidneys contain an extensive network of very small blood vessels.
Therefore, protecting kidney health requires not only blood pressure management but also comprehensive management of cardiovascular and vascular risk factors, including dyslipidemia.
If you have been told during a health checkup that your cholesterol levels are abnormal, please feel free to consult us.
Chronic Kidney Disease (CKD)
Chronic kidney disease (CKD) refers to a condition in which kidney function is reduced or kidney damage, such as persistent proteinuria, continues over time.
CKD often causes few or no symptoms in its early stages.
However, as CKD progresses, kidney function may gradually decline, eventually requiring kidney replacement therapy such as dialysis or kidney transplantation.
CKD is also closely associated with cardiovascular diseases such as heart attack and stroke.
That is why early detection and slowing the progression of CKD as much as possible are so important.
Glomerulonephritis
Glomerulonephritis is a group of diseases in which inflammation occurs in the glomeruli, the structures within the kidneys that filter blood.
There are many different types, including IgA nephropathy.
Glomerulonephritis may be discovered because of proteinuria or hematuria.
If:
“Both protein and blood have been detected in my urine,”
or
“My urine test continues to show blood every time I am tested,”
we recommend undergoing a detailed evaluation by a nephrologist.
Nephrotic Syndrome
Nephrotic syndrome is a condition in which large amounts of protein are lost in the urine, resulting in low levels of albumin in the blood and often causing significant edema.
Because treatment varies depending on the underlying cause, specialized evaluation is important.
Polycystic Kidney Disease
Polycystic kidney disease is a genetic kidney disorder in which numerous cysts develop in the kidneys and kidney function may gradually decline.
It may also be associated with hypertension and hematuria.
If you have a family member with polycystic kidney disease or have been told that multiple kidney cysts were found on imaging, we recommend consulting a nephrologist.
Treatment with tolvaptan (Samsca®) is also available when appropriate.

However, the goal of kidney disease treatment is not simply to “avoid dialysis.”

You may want to travel.

You may have dreams and things you still want to accomplish.
That is why I believe it is so important to detect kidney disease early and slow its progression as much as possible, so that each person can continue living the life that matters to them.
And even if kidney replacement therapy eventually becomes necessary, there are several options, including:
Hemodialysis, peritoneal dialysis, home hemodialysis, and kidney transplantation.
Because our clinic also provides dialysis care, we can think together about not only your current kidney function but also the options available if your kidney function declines in the future.
