あなたの体を守る「透析の原理」と「治療法の特徴」

 

1.透析の基本原理 ― どうやって血液をきれいにしているの?

腎臓の役割を代行する透析治療は、主に「拡散」「濾過(ろか)」「吸着(きゅうちゃく)」という3つの仕組みを組み合わせて行われています。

a. 拡散(かくさん)― 小さな老廃物を取り除く

コップの水にティーバッグを入れると、お茶の成分が自然に広がっていくように、血液中の老廃物は濃度の低い方へ移動します。

血液と透析液を薄い膜(フィルター)を介して接触させることで、尿素、クレアチニン、カリウムなどの老廃物を効率よく取り除きます。

b. 濾過(ろか)― 余分な水分をしぼり出す

コーヒーをドリップするように、血液に圧力をかけて余分な水分を取り除く仕組みです。これを「除水(じょすい)」と呼びます。

さらに、水分とともに比較的大きな老廃物も「対流」という流れによって体外へ排出されます。

c. 吸着(きゅうちゃく)― 膜にくっつけて取り除

一部の特殊なフィルター(PMMA膜など)は、拡散や濾過では取りきれない物質を膜の表面に吸着させる働きを持っています。

かゆみの原因となる物質や炎症に関係する物質を取り除くことで、症状の改善が期待できます。

 

 

2.あなたに合わせた「3つの治療モード」

当院では、検査データやその日の体調、生活スタイルに合わせて、最適な透析方法を選択しています。

① 血液透析(HD)

標準的で安定した透析治療

最も基本的な治療法で、主に「拡散」の力を利用して小さな老廃物を取り除きます。

メリット

・体に必要な栄養(アルブミンなど)が失われにくい

・安定した治療を受けられる

・栄養状態が気になる方にも適している

② オンラインHDF

中〜大きな老廃物まで徹底的に除去

血液透析に「大量の濾過」を組み合わせた治療法です。

血液を大量の置換液で洗い流しながら、老廃物を効率よく除去します。

メリット

・通常の透析では取りにくい中分子量物質を除去できる

・強いかゆみの改善が期待できる

・関節痛の軽減につながる

・レストレスレッグス症候群(脚のむずむず感)の改善が期待できる

③ I-HDF(間歇補充型HDF)

血圧を安定させる「血管へのやさしい刺激」

日本で開発された、血圧の安定を目的とした透析方法です。

透析中、一定の間隔で少量の置換液を自動的に補充します。

メリット

・透析中の血圧低下を防ぎやすい

・足のつりを軽減する

・血流を改善する

・心臓への負担を軽減する

 

3.命を支える透析膜(ダイアライザー)へのこだわり

透析治療で最も重要な役割を担うのが、「ダイアライザー(人工腎臓)」です。

ダイアライザーの中には、髪の毛ほどの細さ(約200〜250μm)のストロー状の繊維(中空糸)が約1万本入っています。

その繊維の壁には、目には見えない無数の小さな孔(あな)が開いており、血液は中空糸の内側を、透析液は外側を流れます。

この孔を通して老廃物や余分な水分を取り除き、体に必要な成分はできるだけ残すことで、腎臓の働きを代わりに担っています。

透析膜は素材によって性能が大きく異なるため、当院では患者さん一人ひとりの体質や症状に合わせて最適な膜を選択しています。

PS膜(ポリスルホン膜)

世界で広く使用されている高性能膜

老廃物を効率よく除去できる現在の標準的な透析膜です。

小さな老廃物から比較的大きな尿毒素までバランスよく除去できるため、多くの患者さんに使用されています。

近年では、生体適合性をさらに高めたビタミンEコーティング膜なども登場し、より体にやさしい透析が可能となっています。

CTA膜・ATA膜(セルローストリアセテート膜)

天然素材由来で体にやさしい膜

綿花を原料とした天然由来の透析膜です。

血液が膜に触れた際の刺激が少なく、生体適合性に優れていることが特徴です。

透析開始時に血圧が下がりやすい方や、アレルギーが心配な方にも適しています。

さらに進化したATA膜は、老廃物の除去性能も高められています。

PMMA膜(ポリメチルメタクリレート膜)

「吸着」で治療する特別な透析膜

PMMA膜は、老廃物を孔から通すだけでなく、膜そのものに吸着させて取り除く特殊な機能を持っています。

炎症に関係する物質や、かゆみの原因となる物質を除去できるため、

・かゆみが強い方

・関節痛がある方

・炎症が続いている方

などで高い効果が期待されます。

PEPA膜(ポリエステル系ポリマーアロイ膜)

高い安全性と優れた生体適合性

特殊な三層構造を持ち、老廃物をしっかり除去しながら、透析液側からの不純物の流入を防ぐ性能に優れています。

血液への負担が少なく、生体適合性にも優れているため、長期間にわたり安定した透析治療を支えます。

 

膜の選択において当院が大切にしていること

透析膜には、「たくさんの老廃物を除去する力」と「体に必要な栄養を守る力」の両方が求められます。

老廃物を多く除去しようとすると、大切なアルブミンまで失われることがあります。一方で、アルブミンを守りすぎると、大きな尿毒素が体内に残ってしまいます。

そのため当院では、

「必要な毒素はしっかり除去し、大切な栄養はできるだけ守る」

という最適なバランスを追求しています。

患者さんの症状や栄養状態、透析効率(spKt/V)などを総合的に評価し、一人ひとりに最適な透析膜を選択しています。

4.透析管理の要 ― DWとKt/V

ドライウエイト(DW)

体の中に余分な水分がない状態の「あなたにとって最適な体重」です。

心胸比や血圧、むくみなどを総合的に評価し、適切な除水量を決定します。

Kt/V(ケーティー・オーバー・ブイ)

透析によって血液がどれくらいきれいになったかを示す指標です。

当院では、患者さんが長く元気に生活できるよう、十分な透析量の確保を大切にしています。

Understanding Hemodialysis

The Principles of Dialysis and the Characteristics of Different Treatment Methods

1. The Basic Principles of Dialysis — How Is Your Blood Purified?

Dialysis replaces some of the functions normally performed by the kidneys. It cleans the blood through three main mechanisms: diffusion, filtration, and adsorption.

a. Diffusion — Removing Small Waste Products

Just as tea naturally spreads when a tea bag is placed in a cup of water, waste products in the blood move from areas of higher concentration to areas of lower concentration.

By allowing blood and dialysis fluid to come into contact across a thin membrane (filter), waste products such as urea, creatinine, and potassium are efficiently removed.

b. Filtration — Removing Excess Fluid

Similar to the way coffee is filtered through a drip system, pressure is applied to the blood to remove excess fluid. This process is called ultrafiltration.

In addition, relatively large waste products are carried out of the body together with water through a process known as convection.

c. Adsorption — Removing Waste by Capturing It on the Membrane

Certain specialized filters, such as PMMA membranes, can adsorb substances onto the membrane surface that cannot be removed by diffusion or filtration alone.

By eliminating substances associated with itching and inflammation, dialysis can help improve symptoms.

① Hemodialysis (HD)

Standard and Stable Dialysis Therapy

Hemodialysis is the most common form of dialysis and primarily uses diffusion to remove small waste products.

Benefits

• Minimizes the loss of important proteins such as albumin

• Provides stable and reliable treatment

• Suitable for patients concerned about their nutritional status


② Online Hemodiafiltration (Online HDF)

Thorough Removal of Medium- and Large-Sized Waste Products

Online HDF combines conventional hemodialysis with a large amount of filtration.

Blood is continuously cleansed using a substantial volume of replacement fluid, enabling efficient removal of waste products.

Benefits

• Removes middle-molecular-weight substances that are difficult to eliminate with conventional dialysis

• May improve severe itching

• May reduce joint pain

• May improve restless legs syndrome


③ Intermittent Hemodiafiltration (I-HDF)

Gentle Support for Stable Blood Pressure

Developed in Japan, I-HDF is designed to stabilize blood pressure during dialysis.

Small amounts of replacement fluid are automatically infused into the bloodstream at regular intervals during treatment.

Benefits

• Helps prevent drops in blood pressure during dialysis

• Reduces muscle cramps

• Improves blood circulation

• Reduces stress on the heart


 

3. Our Commitment to Dialysis Membranes (Dialyzers)

One of the most important components of dialysis treatment is the dialyzer, often referred to as the “artificial kidney.”

Each dialyzer contains approximately 10,000 hollow fibers, each measuring only 200–250 μm in diameter—about the thickness of a human hair.

The walls of these fibers contain countless microscopic pores. Blood flows inside the fibers, while dialysis fluid flows outside.

Through these pores, waste products and excess water are removed while essential substances are retained, allowing the dialyzer to perform many of the functions of the kidneys.

Because membrane materials differ greatly in their properties, we carefully select the most suitable membrane for each patient according to their physical condition and symptoms.

PS Membrane (Polysulfone Membrane)

A High-Performance Membrane Widely Used Around the World

Polysulfone membranes are the current standard in dialysis treatment.

They efficiently remove both small and relatively large waste products and are therefore suitable for many patients.

Recently, vitamin E-coated membranes have been introduced to further improve biocompatibility and provide even gentler dialysis treatment.


CTA and ATA Membranes (Cellulose Triacetate Membranes)

Natural Materials That Are Gentle on the Body

These membranes are derived from cotton-based materials.

They are highly biocompatible and cause minimal blood–membrane reactions.

They are particularly suitable for patients who experience low blood pressure at the start of dialysis or who are concerned about allergic reactions.

The newer ATA membranes provide even greater toxin-removal capacity.


PMMA Membrane (Polymethyl Methacrylate Membrane)

A Unique Membrane That Removes Waste Through Adsorption

Unlike conventional membranes, PMMA membranes remove waste not only through pores but also by adsorbing substances directly onto the membrane itself.

They are particularly effective in removing substances associated with inflammation and itching.

PMMA membranes may be beneficial for patients with:

• Severe itching

• Joint pain

• Persistent inflammation


PEPA Membrane (Polyester Polymer Alloy Membrane)

Excellent Safety and Biocompatibility

PEPA membranes feature a unique three-layer structure that efficiently removes waste products while preventing impurities from entering from the dialysis fluid side.

Because they place less stress on the blood and provide excellent biocompatibility, they support stable long-term dialysis treatment.


Our Philosophy in Membrane Selection

An ideal dialysis membrane must achieve two important goals: removing harmful waste products while preserving essential nutrients.

If too many toxins are removed, valuable proteins such as albumin may also be lost. Conversely, preserving too much albumin may allow larger toxins to remain in the body.

For this reason, our clinic strives to achieve the optimal balance:

We carefully evaluate each patient’s symptoms, nutritional status, and dialysis efficiency, including spKt/V, in order to select the most appropriate membrane.

4. Key Indicators in Dialysis Management — Dry Weight and Kt/V

Dry Weight (DW)

Dry weight is your ideal body weight without excess fluid.

We determine the appropriate amount of fluid removal by comprehensively evaluating factors such as the cardiothoracic ratio on chest X-rays, blood pressure, and swelling.


Kt/V

Kt/V is an indicator of how effectively dialysis removes waste products from the blood.

At our clinic, we place great importance on ensuring an adequate dialysis dose so that our patients can maintain their health and enjoy a long, active life.

Hemodialysis generally requires treatment three times a week, with each session lasting approximately four hours.
